IBS or SIBO? Why Getting the Diagnosis Right Is the Only Way to Actually Fix Your Gut

IBS is one of the most commonly diagnosed and least satisfying diagnoses in medicine. It’s not that irritable bowel syndrome isn’t real. It absolutely is, and the people living with it experience genuine, often debilitating symptoms. The problem is that IBS is a descriptive diagnosis, meaning it describes a pattern of symptoms without identifying what’s actually causing them. Giving someone an IBS diagnosis without investigating the root cause is a bit like labeling a leaky ceiling as ‘water drip syndrome’ instead of finding the hole in the roof. The label doesn’t help you get better. It just gives your symptom a name.

Small intestinal bacterial overgrowth, known as SIBO, is one of the most common and most frequently missed root causes of IBS-type symptoms, and it requires completely different treatment than functional IBS that isn’t driven by bacterial overgrowth. This distinction matters enormously because treating SIBO as though it’s functional IBS not only fails to resolve the problem but can actively make it worse. Understanding the difference between these two conditions, how they’re properly evaluated, and what a root cause approach to gut health actually looks like is where lasting resolution starts.

What SIBO Is and Why It’s So Frequently Missed

The small intestine is designed to have relatively low bacterial counts compared to the large intestine. Its primary job is digestion and nutrient absorption, and a large bacterial population in that space interferes with both of those functions significantly. SIBO occurs when bacteria, either species that belong in the colon or pathogenic bacteria that don’t belong anywhere in the digestive tract in significant numbers, migrate into the small intestine and establish an overgrowth that produces symptoms through fermentation and gas production.

When these bacteria encounter undigested carbohydrates in the small intestine, they ferment them and produce gas, primarily hydrogen or methane depending on the bacterial species involved. This gas production is what causes the characteristic SIBO symptoms:

  • Bloating that often worsens progressively throughout the day and after meals, sometimes visibly distending the abdomen by evening
  • Gas that is excessive, poorly controlled, or foul-smelling and doesn’t correspond predictably to what you ate
  • Abdominal cramping and discomfort, particularly in the hours following meals
  • Altered bowel motility, meaning constipation, diarrhea, or both alternating unpredictably
  • Nausea and early satiety that makes eating uncomfortable
  • Nutrient deficiencies over time, particularly B12, iron, and fat-soluble vitamins, because bacterial competition and gut lining changes impair absorption

According to a systematic review published in the American Journal of Gastroenterology, studies testing for SIBO in patients with an IBS diagnosis find overgrowth in anywhere from 30% to 85% of cases depending on the diagnostic criteria and testing method used. That’s not a small overlap. It means a significant proportion of people who have been told they have IBS have an underlying bacterial overgrowth that hasn’t been identified or treated, which explains both why their symptoms persist and why standard IBS treatments produce limited results for them.

The Three Types of SIBO and Why the Distinction Matters

Not all SIBO produces the same symptom pattern, and the type of gas being produced by the overgrowth determines both how the condition presents and how it needs to be treated. Getting the type right is as important as getting the diagnosis right.

Hydrogen-dominant SIBO is produced primarily by gram-negative bacteria and tends to present with diarrhea-predominant symptoms, significant bloating, and cramping that is often worse in the hours following meals. It has the most straightforward treatment course of the three types and typically responds well to antimicrobial protocols targeting the specific bacteria involved.

Methane-dominant SIBO, now more precisely termed intestinal methanogen overgrowth or IMO in the research literature, is produced by archaea rather than bacteria and tends to present with constipation-predominant symptoms. This is because methane gas directly slows gut motility, meaning the overgrowth is physiologically causing the constipation rather than the constipation being a separate issue. Methane-dominant overgrowth is significantly harder to treat than hydrogen SIBO, requires a different antimicrobial approach, and almost always requires the addition of prokinetic agents to restore normal motility alongside the antimicrobial treatment. Treating it the same way as hydrogen SIBO produces poor outcomes.

Hydrogen sulfide SIBO is the third and least well characterized type in clinical practice. It tends to present with diarrhea, urgency, significant bloating, and symptoms that worsen noticeably with high-sulfur foods like eggs, garlic, onions, and cruciferous vegetables. A characteristic rotten egg odor to gas is a common report. Standard breath testing doesn’t capture hydrogen sulfide production, which is one of the primary reasons this type is significantly underdiagnosed even among doctors who routinely test for SIBO.

How SIBO Is Properly Diagnosed

SIBO is diagnosed through breath testing, which measures the gases produced by bacterial fermentation in the small intestine after a lactulose or glucose challenge. The patient drinks a solution of the substrate, breath samples are collected at regular intervals over 2 to 3 hours, and the pattern of gas production and the timing of peaks in the test results help identify the location and type of overgrowth.

Standard stool testing does not diagnose SIBO. This is one of the most consequential things to understand about SIBO evaluation, and it’s one of the primary reasons SIBO gets missed so consistently in conventional workups. Stool cultures reflect the microbial population in the colon, not the small intestine, and they cannot identify small intestinal bacterial overgrowth regardless of how comprehensive the stool panel is. A negative or normal stool test result says nothing about whether SIBO is present. This is why so many people with SIBO get told their gut testing is normal and are left without answers.

Lactulose breath testing has the advantage of being able to identify overgrowth throughout the length of the small intestine, since lactulose is not absorbed and travels the full length of the small bowel before reaching the colon. Glucose breath testing is sometimes considered more specific for proximal small intestinal overgrowth but misses more distal overgrowth entirely. The clinical picture and symptom pattern inform which test is most appropriate for a given patient.

It’s also worth understanding that breath testing has imperfect sensitivity and specificity. A negative test doesn’t definitively rule out SIBO in someone with a compelling clinical picture. Research published in the Journal of Clinical Gastroenterology has noted that false negative rates for lactulose breath testing can be significant, and experienced functional doctors sometimes treat empirically based on symptom pattern and history when testing is negative but clinical suspicion remains high.

What Actually Causes SIBO

Understanding the drivers of SIBO is essential for anyone dealing with it, not because it helps you treat it yourself, but because it explains why clearing the overgrowth without addressing the underlying cause leads to rapid and predictable recurrence. This is the reason SIBO has a reputation for being difficult to permanently resolve. The antimicrobial treatment addresses the overgrowth. It does nothing about why the overgrowth developed.

Impaired migrating motor complex (MMC) function is the most important structural driver of SIBO and the one most central to understanding recurrence. The MMC is the cleansing wave of muscular contractions that sweeps the small intestine between meals, moving bacteria downward toward the colon where they belong. It only activates during the fasted state, which is why eating frequently throughout the day without adequate fasting intervals is one of the most common and least recognized SIBO recurrence drivers.

The MMC is damaged by a specific mechanism that occurs after food poisoning called post-infectious IBS. According to research published in the American Journal of Gastroenterology, foodborne illness involving certain bacteria triggers an immune response that produces antibodies which cross-react with the nerve cells governing the MMC, effectively damaging the small intestine’s self-cleaning mechanism. This is why some people trace their gut issues back to a single episode of food poisoning that they never fully recovered from.

The MMC is also impaired by hypothyroidism (which slows gut motility broadly), opioid medications, and by eating patterns that keep insulin elevated throughout the day and prevent the MMC from ever fully activating.

Low stomach acid is a significant predisposing factor for SIBO because gastric acid is one of the body’s primary defenses against bacteria surviving long enough in the upper digestive tract to reach the small intestine. Proton pump inhibitors, which are among the most commonly prescribed medications in the United States, dramatically raise gastric pH and are strongly associated with SIBO development and recurrence. Research published in Gut found that PPI users have significantly higher rates of SIBO than non-users, and that risk increases with duration of use.

Structural abnormalities including adhesions from prior abdominal surgery, appendectomy history, ileocecal valve dysfunction, and other anatomical factors that impair the normal directional flow of intestinal contents all predispose to bacterial accumulation in the small bowel by disrupting the mechanical clearance mechanisms the small intestine depends on.

Immune dysfunction reduces the body’s ability to regulate bacterial populations in the small intestine. This is particularly relevant in people with autoimmune conditions, which have their own relationship with gut health, and in people with chronic stress-driven immune suppression.

Hypothyroidism deserves specific mention because it both slows the gut motility that predisposes to SIBO and impairs the immune surveillance that helps keep the small intestine clear. A complete thyroid panel is a standard component of a thorough gut health evaluation precisely because addressing thyroid dysfunction can be an essential part of resolving the motility and immune factors driving SIBO.

Why IBS Without SIBO Has Different Root Causes

When IBS symptoms are present without SIBO on breath testing, the root cause investigation shifts to other potential drivers that require their own distinct evaluation and approach. These are not interchangeable with SIBO treatment, which is exactly why accurate diagnosis matters so much before treatment is initiated.

Gut dysbiosis in the large intestine, where bacterial imbalance drives inflammation, altered motility, and gut lining changes through different mechanisms than small intestinal overgrowth, produces overlapping symptoms but requires microbiome restoration approaches rather than antimicrobial treatment.

Intestinal permeability, where a compromised gut lining allows inflammatory particles to cross into systemic circulation, drives immune activation and systemic inflammation that affects gut function and produces symptoms that extend well beyond the digestive tract. According to research published in Gut, intestinal permeability is significantly elevated in IBS patients compared to healthy controls and appears to be a driver rather than just a consequence of the condition.

Food sensitivities driving immune-mediated gut inflammation are a common and frequently unidentified driver of IBS-type symptoms. Unlike food allergies, which involve rapid IgE-mediated reactions, food sensitivities involve delayed IgG and IgA responses that can take up to 72 hours to produce symptoms, making them nearly impossible to identify without testing.

Visceral hypersensitivity, where the enteric nervous system becomes sensitized and registers normal intestinal sensations as painful, is a well-documented feature of IBS particularly following infections or periods of significant stress. This central sensitization component explains why some people continue to experience significant symptoms even after gut inflammation or dysbiosis has been addressed.

Gut-brain axis dysregulation, where chronic stress, anxiety, or trauma history alters gut motility and pain sensitivity through the enteric nervous system, is present to some degree in nearly all IBS cases and requires its own targeted support alongside any gut-directed interventions.

The Problem With Treating Blindly

One of the most common patterns I see in patients who arrive at my office having already done significant self-directed gut health work is that they’ve tried multiple protocols without testing to confirm what they were actually treating. They’ve done a low-FODMAP diet, tried several rounds of probiotics, eliminated gluten, done a gut cleanse, and still feel terrible. Sometimes things improve slightly for a while and then backslide. Sometimes nothing helps at all.

The reason targeted gut treatment requires accurate diagnosis first is that the interventions for different root causes are not just different, they can be directly counterproductive when applied to the wrong condition. High-dose probiotics, for example, can worsen SIBO symptoms significantly because they add bacterial load to a small intestine that is already overgrown. High-fiber diets that support a healthy large intestine microbiome feed the bacterial overgrowth in SIBO and amplify symptoms. Without knowing what you’re treating, the interventions that help one condition can actively perpetuate another.

This is not a criticism of the people who try these approaches. It’s a reflection of how genuinely complex gut health is and why accurate evaluation by a doctor who understands the full diagnostic picture is the most efficient path to actually feeling better.
